This article describes an algorithm to guide management of hypoxia occurring under anaesthesia. The main learning points are:

·         During anaesthesia low oxygen saturations must be treated immediately

·         Administer 100% oxygen

·         Ventilate by hand, call for help and consider ‘ABCDE’

·         Check for likely causes in a logical sequence

·         Treat causes as you identify them
